Hi, Assume that there is patient which was diagnosed with a chronic disease like "Type 2 Diabetes" in a visit at time T. In that visit, this diagnosis will be recorded in condition table. My question is that, is this diagnosis will be recorded in condition table again when the patient have a new visit at time T + 1?

Hi @egealpay, Great question. Sometimes, but not always. It's going to depend on whether the physician or nurse entered that information into the record during that visit. You shouldn't assume that all relevant information will be available at each individual visit but that you may have to look into the patients history for that information. Hope that helps, @trberg
