Sorry to hear that you are experiencing issues! I am checking the request logs, and I unfortunately do not see your Synapse username (`leonzhou`) among the list of responses. Can you try submitting the form again and ensure that "leonzhou" is entered in the **Synapse Username** question?
If you still do not receive an email after resubmitting the form, try checking your Spam folder or your Synapse email notification settings.
